Driver Application

This form is for drivers of the CDN vans

  • VEHICLE POLICY

    The CDN van(s) are owned and administered by the Community Development Network Trust (CDN). They are available for use, to authorised persons or organisations (hereinafter referred to as the “organisation”) as prescribed by CDN, and subject to their acceptance of the conditions set out below. 


    1. The CDN vans are to be used primarily for:
    2. Transportation of children, youth and their families on CDN programmes/activities 
    3. CDN employees for their daily work
    4. The transportation of people on community building activities

  • CONDITIONS OF USE
    1. The organisation will be charged for an administration fee and mileage at our current rates. See “Costs & Charges” section below.  Rates subject to change without notice.
    2. The fuel card provided must be used when refuelling. The cost of fuel is covered in the mileage charge. The fuel card is for the exclusive use of fuelling CDN vans.  The fuel card will be with the logbook and the pin is 2368 (CDNT)
    3. The driver must hold a valid driver’s license. They must supply this information when gaining approval to drive the van. Written approval from CDN Trust is required prior to driving as only approved drivers are permitted to drive the van. 
    4. Applicants with driving convictions or infringements within the last 7 years may not be approved. If a driver receives any driving conviction or infringement after receiving approval to drive, CDN must be notified and approval to drive may be rescinded.
    5. Driver approval is rescinded with an expired license. the driver must reapply for approval with new license details.
    6. The vehicle must not travel beyond the Canterbury Provincial Boundary unless approval is given at the time of booking.
    7. The maximum belted & seated capacity of the vehicle may not be exceeded. 
    8. No smoking is permitted in the vehicle. 
    9. No alcohol or drugs may be carried or consumed in the vehicle.
    10. No alcohol or drugs are to be consumed by the driver of the vehicle during the usage period or in the same calendar day prior to possession of the vehicle.
    11. Any minor mechanical problems with the vehicle should be noted in the mileage form and reported to CDN administration: office@cdntrust.org.nz or 942-3434 or 027-413-2250
    12. Should an accident or breakdown occur, contact the CDN manager immediately (as soon as safe, practical and must be same-day) on 027 600 3800 (James Harris)
    13. The driver is liable for any traffic or parking  infringement notices incurred and an additional CDN administration fee (see below)
    14. The driver shall ensure that all reasonable care is taken in handling and parking the vehicle and it is left securely locked when not in use. Refer to the clauses below regarding liability and charges. 
    15. It is the responsibility of the organisation borrowing the van to ensure the vehicle is operated in compliance with all relevant laws, their policies and complies with all relevant Health and Safety obligations.  The driver must also abide by all laws and regulations and the policies of CDN Trust and the borrowing organisation.
    16. If any driver is a ‘private borrower’ using the van for personal business rather than on behalf of an authorised organisation, then all responsibilities and charges applicable to the ‘organisation’ in these terms and conditions shall apply to the driver.
  • MAINTENANCE

    The driver must ensure that:


    1. All mileage must be recorded using the online form provided. This requires an entry at the start and end of each van use. Failure to complete required records will incur an administration fee (see below).  cdntrust.org.nz/van

    2. If cleaning is required resulting from use, misuse or negligence while the vehicle is in the organisation's care, the organisation will be invoiced for the full cost of the clean plus an administration fee (see below).

    3. The vehicle is returned with over a quarter-full tank. Use the fuel card provided when refuelling. The cost of fuel is covered in the mileage charge. 

    4. The vehicle is returned clean and tidy.

    6. If undertaking trips outside of Christchurch and/or any issues are observed, the driver also needs to check that: The water in the radiator of the vehicle is maintained at the correct level, the oil in the vehicle is maintained at the correct level, and the tyres are maintained at the correct pressure.


    Any damage or further cleaning required must be reported to office@cdntrust.org.nz or 03-942-3434 or 027-413-2250 immediately on return.

    All other vehicle maintenance is to be conducted by a CDN Trust staff member.

  • INSURANCE

    The vehicle is fully insured. 

    1. The organisation is responsible for paying any excess payable by CDN to the insurer, as a result of any claim made, for any loss or damage to the vehicle while in possession of the organisation.  
    2. If the vehicle is operated in a manner whereby the insurer disallows any claim, or it is operated outside the terms of use stipulated in this document, the organisation shall be liable to pay for the full amount of damage. 
    3. CDN Trust will invoice the organisation for the full cost of loss or excess (in the event of a successful insurance claim) and it is the responsibility of the organisation to recover costs from the driver at the discretion or policy of the organisation.  This applies to any damage to the vehicle while in possession of the using organisation.  It is the responsibility of every driver to enquire and understand the policy of the organisation they are driving on behalf of.

  • CURRENT COSTS & CHARGES

    Charges are subject to change without notice - for current charges refer back to this form.  


    Usage charges:

    1. $7.50 admin fee per booking (for clarity, recurring bookings incur this charge for each use.  E.g. a weekly booking incurs this charge each week)
    2. $0.85 per km (this includes the cost of fuel)

    Damage, Infringements & Insurance:


    Insurance charges:

    1. Glass claims – Nil Excess
    2. For each and every claim - $500 Excess (gst inclusive)
    3. Additional excess when being driven by persons under 25 years of age - $500 Excess (gst inclusive)

    Other Administration fees:


    1. CDN administration fee for any claim - $250
    2. CDN administration fee for any infringements (e.g. tickets or legal charges) - up to $100
    3. CDN administration fee if mileage forms are not completed - $50
    4. CDN administration fee if additional cleaning is required - $50 (in addition to cleaning costs)

    All rates stated are gst exclusive unless otherwise specified.

    When vehicle is used by external organisation, CDN will invoice the organisation for these costs/fees. It is at the discretion of the organisation whether to recover costs from the driver.

  • DRIVERS LIABILITY ON CDN TRUST PROGRAMMES

    This clause applies ONLY to CDN volunteers & CDN staff during CDN Trust programmes or on other CDN business:


    For drivers of the CDN van(s) during CDN programmes or on other CDN business the following will apply in the case of accident, damage or loss:


    1. In the event of an accident which is deemed by the police, insurance company or CDN board of trustees to be the fault of the driver, the driver is liable to pay $200 excess. 
    2. If the vehicle is operated in a manner deemed by police or the CDN board of trustees to be reckless, illegal, outside the terms of use stipulated in this agreement, or outside the instructions given to the driver by the CDN programme director, then the full insurance excess will be payable by the driver. 
    3. If due to any of the above, which is deemed to be the fault of the driver, insurance cover is declined, then the full costs will be the full responsibility of the driver.
    4. If there is damage to the vehicle which is not deemed to be the fault of the driver then the driver will not be liable for the costs or excess.
